A collaborative community effort connecting older adults and their caregivers with social engagement, healthcare screenings, therapeutic programs, resources, and ongoing support.
The Ward Four Senior Social Hub is part of The Washington Home's Ward Four Collaboration Grant. Key participants include the Genevieve N. Johnson Senior Center (GNJSC), Iona Senior Services (IONA), Vida Senior Centers (VSC), So Others Might Eat (SOME), Brookland Senior Day Center, Plants and Blooms Reimagined, Story Tapestries, and East Rock Creek Village (ERCV).
The purpose of the collaboration is to engage stakeholders in solution development by improving efficiency and reducing duplication, encouraging cross-sector partnerships, and spurring innovation to align targeted services and outcomes for older adults with The Washington Home's priorities and goals.
This community outreach effort brings together stakeholders from the public and private sectors and foundations focused on senior services in Ward 4. Through collaboration, the Hub helps older adults learn about and benefit from programs, services, and initiatives available throughout the community.

The Washington Home invested $238K to build a Senior Social Hub for older adults in Ward Four.
This unique collaboration unites eight community-based organizations to enhance social connection programming, improve therapeutic care access, and expand cognitive health screenings for early detection and support.
The Senior Social Hub is a coordination effort among eight community-based organizations working together to strengthen program support and healthcare screenings for older adults across Ward Four. The goal is to reduce social isolation, improve health outcomes, and expand access to critical services for older adults.
The investment will support an integrated network of senior service organizations and community partners that will collectively serve more than 3,300 older adults through therapeutic recreational services, preventive health screenings, and social engagement opportunities for ongoing care and support.

The Washington Home is proud to partner with eight organizations delivering multiple points of support across the health care continuum for older adults.
Provides adult daycare health services with social, medical, and recreational programs.
Connects members with information, cultural and social activities, education, services, and support to help community members confidently age in place.
Provides adult daycare health services with social, medical, and recreational programs, as well as Extended Daycare, Music Memory Cafe, and Caregivers Lounge programs.
Operates a Dementia Navigators program with follow-up after screenings and support for families, along with caregiver respite, an information hotline, and social programming.
Offers nature-based therapeutic programs using plant and flower activities to promote social connection, mental wellness, and engagement, including pop-up Bloom Mobile events.
Provides permanent affordable housing, food, nutritional supplements, medical and safety equipment, healthcare screenings, summer camp, and other social programming.
Facilitates art therapy workshops that promote communication, creativity, and emotional well-being.
Provides a gathering place for Latino seniors, with food security, health information, education, and social interaction that supports aging as part of a community.
Each collaboration partner brings specialized programs, services, and community relationships. Together, the organizations create a coordinated network designed to expand access, strengthen referrals, reduce barriers, and help older adults and caregivers connect with the support they need.
The Washington Home (TWH) is a private foundation that provides funding to entities that create and deliver innovative, compassionate, and well-managed programs to improve the quality of life for elderly and/or terminally ill residents in the Washington, DC metropolitan area.
Founded in 1888, The Washington Home historically provided long-term care through its nursing home facility in upper Northwest Washington, DC, hospice care to patients in their own homes, and care in the in-patient hospice wing of the nursing home.
In recent years, its Board of Directors repositioned the organization by ceasing hands-on care and consolidating its financial resources to be deployed for maximum impact in accordance with its longstanding mission.

The Ward 4 Senior Social Hub Collaborative is part of TWH's redesigned FY26 grant strategy, which prioritizes collaborative solutions that improve coordination, reduce duplication of services, and create sustainable community impact. In FY26, TWH awarded more than $4.5 million to organizations serving older adults across all eight wards of the District of Columbia through initiatives focused on healthcare access, social connection, workforce development, and aging in place.
More than 2,900 individuals will receive preventive health screenings and support services, including cognitive and memory screenings, referral services, medical access, and daycare services.
Approximately 375 older adults will participate in social, educational, and wellness activities that strengthen community connections while increasing awareness of available health resources and screening opportunities.
Integrated screening opportunities and social activities can help reduce social isolation while supporting earlier identification of health concerns that could contribute to poorer health outcomes.
